Description

MELD at Trinity House Homeless Shelter provides housing for homeless pregnant or parenting young mothers and their children for up to 120 days. Services include Three daily nutritionally balanced meals, Needs assessment, Case management, Educational assessment, Life Skills Education, Parenting Education, Referral to mental health nd substance abuse resources, Referrals to primary health care services, and Transportation.

Intake Requirements

Requirements: Homeless, pregnant and/or parenting women between the ages 16 – 21. For intake: OVER THE PHONE INTERVIEW AND WHEN PLACEMENT BECOMES OPEN THEY WILL CALL THEM TO COME INTO THE SHELTER AND CONDUCT INTAKE PROCEDURE
