Daytime Support Center:
Breakthroughs Daytime Support Centers connect adults to a variety of resources including food, safe space, showers, laundry, clean clothing, access to computers, and more. Individuals are connected to medical and mental health resources, employment training, and housing assistance with the goal of reducing crisis and homelessness while building stability.
Breakthroughs Transitional Housing Program works alongside men and women experiencing homelessness to secure permanent or more stable housing and increase income within a 120-day goal through strong case management and connections to resources that address barriers including employment, substance abuse, mental and medical health needs, and legal aid. This housing-first, trauma-informed program provides a dedicated bed, care coordination, and housing retention support.
